Can this be used in DIY sugar scrubbs and body butters?

You can find the max safe usage limits in the IFRA document on each product page -- Category 9 for scrubs, Category 5A for leave-on lotions/butters applied by hand.

Has the formula been changed at all? The last bottle I received smelled more fruity and slightly different than my previous bottles. We've had this scent for many years (15+?).

This fragrance oil has not undergone any formulation changes as far as we know. Variance between batches is common and normal due to ingredients and ingredient sourcing; this is why it's always recommended to small-batch test even if using the same products and recipe.

Can you use this in lip blam?

No, it cannot. Section 1 of the IFRA Guideline on the product page will give you a Lip Usage percentage.
